Health and Social Care Alliance Scotland update - 20 November

ALLIANCE Update

The ALLIANCE is delivering a two year programme that will explore how the GP practice team can support people to live well in their community.
The Links Worker Programme, a partnership between the ALLIANCE and General Practitioners at the Deep End (The Deep End).  Delivery partners include SAMH and the Royal College of General Practitioners. The programme aims to support people to live well through strengthening connections between community resources and primary care.

The programme will see the creation of a new ‘Community Links Practitioner’ role within the GP practice team and will also see existing practice staff becoming skilled in identifying local services and matching them to the needs of individual patients.  Read more
